About the Locality Teams

Locality teams are responsible for assessing individuals who appear to have an eligible need under the Care Act and:

• Needs are likely to be longer term; or
• Timescales for meeting eligible needs is likely to be longer than a basic assessment and care planning process;
• They require Court of Protection or other legal intervention;
• There are multiple presenting issues such as physical and mental ill health, substance abuse plus difficult to engage;
• An MDT has taken place and longer-term involvement is required eg housing issue;
• They have a terminal diagnosis or palliative needs;
• They have a later stage degenerative illness where change is regular or continuous;
• Attendance at a CHC MDT is required (unless SPA worker involved in a case);
• There are complex family dynamics;
• There is a need for ongoing risk management.

A holistic approach is applied to care and support planning to ensure medium to long term needs/ risks/ outcomes are addressed.
