About the Team - ACT
The team vision is "Achieving sustainable outcomes for Gateshead residents by promoting active, healthy, inclusive, independent lifestyles". The ACT team will review service users with the aim of working closely with these people to promote independence, which in turn will result in less dependency on long-term statutory services.
To deliver on the vision the ACT team will focus on what people can do rather than focussing on their diagnosis or what they can't do. Taking this approach and building on the assets within individuals, will aim to offer more affordable solutions and better outcomes for individuals. The objectives of the ACT team are to work with identified cohorts of service users and will carry out in-depth reviews to consider the whole person, their life story, family, social networks, environment, health and wellbeing. Will also establish if enablement or technology enabled care can be utilised to support an individual.
