About the Team - ASSET
The Adult Service Support and Engagement Team (ASSET) focus their attention on proactively promoting wellbeing and independence, helping individuals take greater control over their lives, by championing an early intervention model which allows them to access other agencies and provides them with the tools to be both physically and mentally healthy.
The ASSET team can work with people over 18 who are:
- Requiring support to maintain their overall health and wellbeing;
- Need support to engage in meaningful activities;
- Feeling at risk of exploitation;
- Experiencing problematic drug and/or alcohol use;
- In need of support to access appropriate housing services;
- Self-neglecting including hoarding;
- Requiring support to engage with other agencies.
The ASSET team can support people with:
Health and Wellbeing: Working with people to better understand how to be physically and mentally healthy.
Positive Relationships: Supporting people to build positive and appropriate relationships with friends, family, and associates.
Safe & Secure Home: Supporting those who have housing issues, are homeless or at risk of homelessness.
Access to Local Communities & Services: Supporting people to access local support and services depending on their needs, interests, and goals.
We provide early intervention and work with other agencies and services to provide:
- Information, guidance, and advice: For our partner agencies on appropriate services;
- Signposting: For individuals to access support via organisations, agencies, and community assets;
- Allocated Cases: Where there is specific work needed with individuals;
- Referrals to statutory services if needed;
- Involvement in partner led Multi-Agency Risk Meetings for individuals where there is greater risk or more complex issues.
