About the Team - QEH Social Work

The QEH Social Work team facilitates all discharges from hospital for service users aged over 18 years old who reside in the Gateshead area.

Currently, we are operating as a service using the discharge to assess model to facilitate hospital discharges alongside Covid guidance for hospital discharge. This means all service users are discharged from hospital with a short-term service in line with government guidance for discharge to assess model.

Our team is currently split into two functions; the first function is to facilitate hospital discharges and the second is to oversee discharges for up to a period of six weeks. This work involves overseeing service users in either further assessment beds or within the home environment to monitor and review their level of need and what, if any, ongoing support may be required to meet that need. The legislation we operate within when completing assessments includes the Care Act 2014, MCA 2005, MHA, DOLS, Human Rights Act.
